This analysis of data from four clinical trials found that a cut-off score of less than five on the MADRS and less than four on the YMRS corresponded with an objective global clinical measure of remission in patients with bipolar disorder. Though lower than conventional cut-off scores, these empirically based cut-off points may be more clinically meaningful.

In this study, carried out on a large sample of psychiatric patients, the MDQ was used to screen for bipolar disorder and the SCID-I was used to corroborate the diagnosis. A correct diagnosis was by far the most important prerequisite for adequate treatment of bipolar disorder, and outpatients received adequate treatment markedly less often than inpatients. These findings suggest that adequacy of treatment of bipolar depression is a major concern in outpatient settings.
