In a skilled nursing facility, in 49 women aged 68 to 100 years, 60% (95% CI 46-74%) had 25-hydroxyvitamin D levels of 20 ng/ml or less, which is associated with secondary hyperparathyroidism; only 4% of women had levels above 30 ng/ml, recently recommended as optimal. Vitamin D status was suboptimal even in most women taking multivitamins. The high prevalence of vitamin D deficiency, even in those women taking multivitamins, indicates that practical new approaches for vitamin D repletion in this population are urgently needed.

Centenarians, 104 subjects over 98 years of age, were studied to evaluate possible interventions to avoid fragility fractures. Serum 25-hydroxyvitamin D was undetectable in 99 of 104 centenarians. Thus, the extreme decades of life are characterized by a vitamin D deficiency, low serum calcium, and secondary hyperparathyroidism with an increase in bone resorption and severe osteopenia. There is a potential prevention of osteoporotic fractures and fracture-induced disability through the supplementation with calcium and vitamin D.

Pediatrics 2003, 111:908-910. Rickets in infants attributable to inadequate vitamin D intake and decreased exposure to sunlight continues to be reported in the United States. It is recommended that all infants, including those who are exclusively breastfed, have a minimum intake of 200 IU of vitamin D per day beginning during the first 2 months of life. In addition, it is recommended that an intake of 200 IU of vitamin D per day be continued throughout childhood and adolescence, because adequate sunlight exposure is not easily determined for a given individual. These new vitamin D intake guidelines for healthy infants and children are based on the recommendations of the National Academy of Sciences.

All patients with persistent, nonspecific musculoskeletal pain are at high risk for the consequences of unrecognized and untreated severe hypovitaminosis D. This risk extends to those considered at low risk for vitamin D deficiency. Nonimmigrant women of childbearing age with such pain appear to be at greatest risk for misdiagnosis or delayed diagnosis. Because osteomalacia is a known cause of persistent, nonspecific musculoskeletal pain, screening all outpatients with such pain for hypovitaminosis D should be standard practice in clinical care.

A recent review evaluated and summarized the evidence of muscle weakness as a risk factor for falls in older adults. Thirty studies met the selection criteria; data were available from 13. For lower-extremity weakness, the combined OR was 1.76 (95% confidence interval (CI) = 1.31-2.37) for any fall and 3.06 (95% CI = 1.86-5.04) for recurrent falls. For upper-extremity weakness the combined OR was 1.53 (95% CI = 1.01-2.32) for any fall and 1.41 (95% CI = 1.25-1.59) for recurrent falls. The authors conclude that muscle strength (especially lower extremity) should be one of the factors that is assessed and treated in older adults at risk for falls. More clinical trials are needed to isolate whether muscle-strengthening exercises are effective in preventing falls.

The goal of this study was to assess the effectiveness of vitamin D in preventing an older person from falling; 5 of 38 identified studies were included in the primary analysis. Fifteen patients would need to be treated with vitamin D to prevent 1 person from falling. Vitamin D supplementation appears to reduce the risk of falls among ambulatory or institutionalized older individuals with stable health by more than 20%.

Four monthly supplementations with 100,000 IU oral vitamin D may prevent fractures without adverse effects in men and women living in the general community.

The potential benefits of fruits and vegetables, meat, Ca, vitamins D and K, and phyto-oestrogens were presented and discussed. Other lifestyle factors were also discussed, particularly the effect of physical activity and possible gene-nutrient interactions affecting bone health.

The panel concluded that: (1) osteoporosis is a disease that requires continuing medical attention to ensure optimal therapeutic benefits; (2) when given in appropriate doses, calcium and vitamin D have been shown to be pharmacologically active (particularly in patients with dietary deficiencies), safe, and effective for the prevention and treatment of osteoporotic fractures; (3) calcium and vitamin D are an essential, but not sufficient, component of an integrated management strategy for the prevention and treatment of osteoporosis in patients with dietary insufficiencies, although maximal benefit in terms of fracture prevention requires the addition of antiresorptive therapy; (4) calcium and vitamin D are a cost-effective medication in the prevention and treatment of osteoporosis; (5) it is apparent that awareness of the efficacy of calcium and vitamin D in osteoporosis is still low and further work needs to be done to increase awareness among physicians, patients, and women at risk; and (6) in order that calcium and vitamin D continues to be manufactured to Good Manufacturing Practice standards and physicians and other health care professionals continue to provide guidance for the optimal use of these agents, they should continue to be classified as medicinal products.
