Eur Heart J 2007; 28:1326-1333. This is a recent prospective study in myocarditis patients admitted at a tertiary referral center. Using current immunohistochemical, immunological, and virological tools to establish etiopathogenetic diagnosis, it was found that the majority of cases were immune-mediated. Viral genome was a univariate predictor of adverse prognosis. Biventricular dysfunction at presentation was the main predictor of adverse outcome.

Nat Genet 2007; 39:1074-1082. This is an important study providing evidence for involvement of the IL2RA region, previously associated with autoimmune thyroid disease, in T1D susceptibility. Furthermore, IL2RA susceptibility alleles were correlated with lower circulatory levels of the biomarker soluble IL-2RA, suggesting that an inherited lower immune responsiveness predisposes to T1D.

Frustaci A, Verardo R, Caldarulo M, et al. Myocarditis in hypertrophic cardiomyopathy patients presenting acute clinical deterioration. Eur Heart J 2007; 28:733-740. This is an intriguing yet interesting study describing biopsy-proven viral myocarditis in association with acute clinical deterioration in a monogenic sarcomeric disease, hypertrophic cardiomyopathy. If independently confirmed by others, it could provide an interesting link between observations in animal models and human disease. Genetically diseased myocardium could be more susceptible to viral and/or immune-mediated damage.

A novel fluorescence method for the rapid detection of functional β1-adrenergic receptor autoantibodies in heart failure
-
Using FRET, sera classified as antibody-negative by previous methods were found to contain low-activator antibodies. In addition, it was reported that β-blockers were not able to prevent antibody-induced receptor activation, but this was accomplished by blocking peptides
