The results of this protocol biopsy study found that, during the first year after transplantation, low levels of the CNI tacrolimus were associated with CAN whereas higher tacrolimus levels did not correlate with lesions of CNI toxicity. Thus, these data suggest that during the first year after transplantation acute rejection from low tacrolimus levels may be more important than potentially toxic high tacrolimus levels in causing CAN
