This paper examines to what extent dysfunction in basic perceptual and automatic processes may contribute to the cognitive deficits and symptoms of schizophrenia. Starting in the middle of the last century, phenomenological literature (predominantly German, e.g. Matussek, Conrad, Binswanger and Blankenburg) described changes in perception and automatic processes that are related to the altered experience of self in schizophrenia. Subsequent empirical and experimental research (reviewed in the paper) supports this view. An appendix describes some of the basic concepts of Gestalt psychology as well as the phenomenology of perception and automatic processing.

Interestingly, that part of the anterior cingulate cortex (the rostral caudal zone), identified by the same laboratory in numerous previous studies to be associated with error/performance monitoring, did not differentiate between aware and unaware errors. These findings lend support to the emerging view that the 'control' (or monitoring) of conscious, controlled effortful-strategic processing - thought to be mediated by anterior cingulate activity and found to be reduced in patients with schizophrenia - does not itself require awareness.

Uhlhaas P, Linden DEJ, Singer W, et al. Dysfunctional long-range coordination of neural activity during Gestalt perception in schizophrenia. J Neuroscience 2006; 26:8168-8175. This study found phase synchrony deficits in the Beta-band in patients with schizophrenia during Gestalt perception, suggesting that long-range cortical coordination is compromised in these patients. Synchronization of neural oscillations acts to integrate highly distributed networks into a coherent assembly, thereby enabling the perception of a Gestalt. The reduced synchrony is thought to index disrupted binding underlying impaired Gestalt perception in schizophrenia.
