Cochrane Database Systematic Rev 2001; (1):CD001800. •• In this meta-analysis involving more than 8440 patients from 51 studies, the impact of exercise-based cardiac rehabilitation on mortality was assessed in middle-aged patients with CHD. Total mortality and cardiovascular mortality were found to be reduced by 27% and 31%, respectively, as a result of the exercise training intervention. This was not the result of either a decrease in nonfatal myocardial infarctions or a decline in rates of sudden cardiac death.

BMJ 2004; 328:189-195. ••In the exercise training meta-analysis of trials in patients with chronic heart failure (ExTraMATCH), the effects of exercise training on survival in patients with heart failure owing to left ventricular systolic dysfunction were assessed. Data sets from nine trials in which a total number of 801 patients had been randomly assigned to exercise training or control were analyzed. Exercise training significantly reduced mortality in patients with chronic heart failure (hazard ratio 0.65, 95% confidence interval 0.46-0.93, P<0.05). In contrast to the anticipation that exercise training might be harmful in patients with chronic heart failure, these data rather imply a reduction in mortality and morbidity in response to supervised aerobic exercise training in these patients.
