Mangano DT, Tudor IC, Dietzel C. The risk associated with aprotinin in cardiac surgery. N Engl J Med 2006; 354:353-365. A total of 4374 adult patients undergoing revascularization surgery were prospectively assessed in an observational multicentre, multicontinental study. A total of 1295 patients received aprotinin, 883 aminocaproic acid, 822 tranexamic acid, and 1374 no antifibrinolytic therapy. Overall, aprotinin significantly increased mortality and the risk for cerebrovascular, cardiovascular, and renal adverse events. After propensity adjustment, only renal events remained significantly higher in the aprotinin-treated patients. Given the large difference in significance before and after propensity adjustment, and the complete lack of control of unknown confounders, the results of the study must be appreciated with caution. Nevertheless, this study will lead to a significant decrease in the use of aprotinin in adult and paediatric cardiac surgical patients, last but not least due to considerable attention provoked in the legal community.

Lancet 2005; 366:556-562. Randomized controlled trial in 116 neonates, infants, and children with CHD and complement component C4A deficiency undergoing open heart surgery. The patients were assigned to either C4A-free or C4A-rich fresh frozen plasma priming of the CPB circuit. Compared with 97% in the C4A-free priming group, only 5% of the patients developed a capillary leak syndrome in the C4A-rich priming group. Two of the C4A-free priming patients died of renal and respiratory failure on day 3 after CPB.

Forty-two neonates undergoing either a Norwood procedure or two-ventricle repair of interrupted aortic arch and ventricular septal defects were randomly assigned to receive a continuous infusion of T3 or placebo for 72 hours after CPB. Patients assigned to hormone treatment had shorter median time to negative fluid balance (2.0 versus 2.5 days, P = 0.027) and higher systolic blood pressure, whereas cardiac index, heart rate, and diastolic blood pressure were not influenced.
