Arch Dis Child 2006; 91:226-232. This is an important paper published in general paediatrics. In this prospective, randomized study, the authors compared two solutions (normal saline and half normal saline, both with 2.5% dextrose), in children hospitalized for acute gastroenteritis. The children were subsequently divided in hyponatraemic or normonatraemic at baseline. In children receiving hypotonic solution (half intracellular fluid), mean plasma sodium remained unchanged in hyponatraemic children (n = 16) and fell 2.3 ± 2.2 mmol/l in normonatraemic children. In contrast, in children receiving isotonic solution (normal saline), the plasma sodium concentration increased by 2.4 ± 2 mmol/l in hyponatraemic children (n = 21), while it remained unchanged in the normonatraemic children. In children having a prolonged fluid infusion (>24 h), three out of eight were hyponatraemic in the half normal saline group compared with none in the normal saline group. In conclusion, in gastroenteritis, normal saline has protective effects against the development of hyponatraemia. The question that arises from these results is: does this recommendation apply only to gastroenteritis or can it be broadened to surgical children?
