The author reviewed and summarized 12 studies of 1133 children with HSP in eight different countries from 1981 to 1990 and reported that 97% of patients experienced renal involvement within 6 months of diagnosis.

The authors retrospectively reviewed the charts of 155 children with HSP. Seven patients had hypertension at long-term follow-up, and two of these patients did not have any evidence of significant renal involvement on presentation. These data support the need for long-term regular blood pressure monitoring in all children with HSP.

The renal function of 73 patients and biopsy specimens from 40 of 73 children with HSP nephritis was reviewed. Patients with lower glomerular filtration rates and higher levels of proteinuria had more severe morphologic changes; however, patients with mild to moderate proteinuria were also found to have severe morphologic changes, which suggests the need for earlier biopsy in this group.

In this study, levels of immunoglobulin-G and immunoglobulin-A ANCA were studied in HSP patients and disease control individuals (13 patients with other vasculitides: PAN, acute urticarial vasculitis, unclassified vasculitis, hypocomplentemic vasculitis). Significantly, more HSP patients had immunoglobulin-A ANCA and lacked immunoglobulin-G ANCA during the acute phase of disease, which suggests that they may serve a role in the diagnosis of HSP.

Treatment without high-dose aspirin in the acute stage of Kawasaki Disease seemed to have no impact on the IVIG response rate or the rate of aneurysm formation: approximately 5% of patients required a second dose of IVIG, and the incidence of coronary aneurysms was 4.14%, with a significantly higher incidence in the IVIG-resistant group (P = 0.033, 2.92% compared with 25%).

In this study, 86.4% of patients presented with constitutional symptoms, 74.5% with skin lesions, 50.9% with fever, 17.3% with gastrointestinal symptoms, 11.8% with renal symptoms, 14.5% with nervous system symptoms, 14.5% with hypertension, and 85.7% with musculoskeletal features. During a median follow-up time of 48 months, remission was reported in 62.9% of patients, and persistent disease was reported in 23.6%, with one report of mortality and two reports of ESRD in this group, which suggested a better renal survival and lower relapse rates in comparison with adults.
