This paper describes a large population-based intervention study in which men showed greater improvements in insulin sensitivity than women in response to endurance exercise training in the absence of weight loss, despite a generally highly variable response. It provides the basis for investigating the mechanisms responsible for the difference between men and women and the variable responsiveness in general, possibly identifying the cause for 'exercise resistance'.

This work demonstrates that boys are more insulin sensitive than girls despite careful control for multiple confounding variables, which suggests that girls are genetically programmed to be more insulin resistant than boys, which explains the often observed higher prevalence of type 2 diabetes in girls than boys.

In this work microarray analysis was used to discover differences in gene expression in livers from male and female rats. Significantly different expression levels were confirmed by reverse transcriptase-polymerase chain reaction and compared with the expression levels in liver tissue from men and women. The most remarkable 18-fold difference in FAT/CD36 expression in male and female rat livers was also present in human livers but to a much smaller extent (approximately twofold).
