The authors provide a case example and an excellent review of this specific and not too rare neurologic complication following multisegment decompression for cervical myelopathy from either an anterior or posterior approach. Previously, non-operative management had been strictly advocated, resulting in partial or complete resolution of symptoms in weeks to months. These authors performed foraminotomies successfully to treat their patient who developed bilateral C5 and C6 dissociated motor radiculopathy following anterior/posterior decompression and fusion.

J Bone Joint Surg Am 2004; 86:1248-1253. Excellent clinical study demonstrating the superior efficacy of MEP neuromonitoring over SSEPs. The authors report a series of 427 patients treated with decompression for either spondylotic myelopathy or ossification of the posterior longitudinal ligament. Two patients developed postoperative neurologic deficits (0.47%). In one patient, SSEP signals remained normal throughout the case, and in the other, the SSEP change lagged the MEP change by 33 minutes. MEP was 100% sensitive and specific.

This article provides a good discussion about ankylosing spondylitis and its effect on cervical spine surgery. Specifically, long posterior constructs are needed. Nine percent of patients developed a new neurologic deficit following surgery.
