Am J Respir Crit Care Med 2004, 169:1001-1006. This is the first study to use stereologic methods to assess airway smooth muscle size and number in nonfatal cases of asthma. Biopsies obtained from the central airways of cases of mild asthma and from control cases were compared using a method of fixation and sectioning of the biopsy that ensures truly unbiased counting of tissue structures-isotropic, uniform, and random orientation. The authors did not find any differences between controls and cases of asthma with regard to the expression of genes associated with smooth muscle phenotype.

In this study, the features of remodeling (and inflammation) that might be characteristic of severe asthma were assessed by comparing pathologic parameters between patients with asthma of varying severity and nonasthmatic controls using multivariate analysis. The results showed that airway smooth muscle cell hypertrophy, assessed by cell width, and reticular basement membrane thickness were markers of severe asthma.

This recent study showed that ASM cells cultured from biopsies from patients with asthma lacked a transcription factor (C/EBPα) that inhibits proliferation. This was not seen in control cases. Transfection of the cells with a viral construct containing message for C/EBPα resulted in growth characteristics of the asthmatic cells that resembled those of cultured ASM cells from control subjects.

This study examined smooth muscle cells from human bronchi. It found that the presence of mRNA for both α and β isoforms of the thromboxane A(2) receptor correlated with protein expression evaluated by radioligand binding. They investigators used an agonist to show that the receptor was functional, mediated an increase in DNA synthesis, and markedly potentiated the mitogenic effect of epidermal growth factor.

This interesting study used a heat-sensitive, retroviral-transduced preparation to examine the characteristics of cultured smooth muscle cells that were growth-arrested without starving them of substrate necessary for growth, as seen in serum deprivation experiments. They showed a predominantly posttranscriptional upregulated expression of contractile proteins in growth-arrested cells. They used microarray to identify a large number of upregulated genes that fell into the following functional groups: translational control (three genes), cell cycle control (seven), extracellular matrix/integrins (four), inflammation (nine), signal transduction (two), and miscellaneous (six).

This study used serum-starved cultured cells to examine the intracellular signals that control the accumulation of contractile proteins in ASM cells. The results showed that phosphotidylinoitide- 3 kinase and the downstream target kinase, mammalian target of rapamycin, activate a further kinase, p70S6, to allow accumulation of contractile proteins.

Clin Exp Allergy 2003, 33:1389-1397. This study compared cultured fibroblasts from patients with asthma with those from nonasthmatic controls. Although they found no difference between these groups, they showed that interleukin-4, which is important in allergic airway inflammation, upregulates procollagen I by direct promoter activation, and that it also increases the TIMP-2:MMP-2 ratio-both profibrotic effects.

This article describes a series of studies using a mouse model and airway biopsy tissues from humans with and without allergic asthma. It demonstrates very nicely that circulating precursors of fibroblasts may traffic to the airway after allergen challenge. Similar observations have also been made in blood vessels. The relative importance of migration versus in situ proliferation in situ as mechanisms to populate the airway wall remains to be determined.

This study shows that, in addition to results of previous experiments from the same group that showed an increased proliferative response of airway smooth muscle cells from cases of asthma, these same cells produce extracellular matrix proteins that are different from those produced by control cells and that themselves have an enhanced proliferative effect on cultured airway smooth muscle cells. Therefore, the increased proliferation of smooth muscle cells from cases of asthma may be a result in part of an autocrine effect of the extracellular matrix proteins they produce. Given the interesting findings from the same laboratory regarding C/EBPα [18••], further results exploring these mechanisms are awaited with interest.

In this study of the peripheral airways of fatal cases of asthma, the investigators have shown that, compared with control cases, there is considerable disruption of elastic tissue in the outer airway wall associated with damage to the surrounding, but not more remote, lung parenchyma. This observation raises a number of interesting issues. If this is a primary event related to asthmatic airway inflammation, it might lead to altered mechanical properties of the lung parenchyma because of the loss of tethering of airways. Through the same mechanism, the airway may be more liable to collapse. A response to this might be to lay down collagen to resist airway narrowing. Therefore, a primary inflammatory process might lead to a remodeling process through mechanical stimuli.

It also highlights the caveats that were put forward with this theory [5], namely that these effects will occur providing the mechanical properties of the airway wall and the ASM are unchanged. Further studies along these lines are eagerly awaited.

These researchers grew bovine ASM cells on flexible silastic membranes, which were then subject to strain. They used this model to investigate the effects of extracellular matrix proteins, strain and dexamethasone on ASM proliferation when incubated with basic fibroblast growth factor. Culture on collagen (but not laminin) enhanced the proliferative effect of strain and this was inhibited by dexamethasone. The study shows the usefulness of this model for examining the single or combined effects of a number of stimuli, or inhibitors, of cell growth.

This study investigated the relationship of ASM cytoskeleton and mechanical strain imposed on the surface of the smooth muscle cell by applying a twisting motion to ferrimagnetic beads glued to the cell surface. It demonstrates the potential role of heat shock proteins in transmitting external forces to the cell cytoskeleton. In this experiment, phosphorylation of heat shock protein 27 led to reduced bead motion, probably because of stabilization of the actin cytoskeleton.

This study showed a link between cell damage and airway inflammation and remodeling by studying the relationship of a remodeling cytokine interleukin-13 and adenosine, which is released during tissue damage. The effect of interleukin-13 overexpression on levels of adenosine, adenosine deaminase activity, and adenosine receptors was examined. The results showed that interleukin-13 and adenosine stimulated one another to amplify their individual effects. This study provides a model of tissue damage and remodeling in the setting of acute inflammation, as discussed in the accompanying editorial [45].

This study did not demonstrate a change in overall basement membrane thickness or in lung function, although was not powered to do so because only 24 subjects were studied over a 10-week period, during which they received three injections of mepolizumab.
