    • The authors present one of the first studies to investigate the spatial organisation of sensory input and motor output in patients with focal hand dystonias. They use TMS to evoke EMG responses in a selection of hand muscles and show that the usual pattern of effects elicited from stimulation of different nerves in the hand is disrupted in dystonia.

    • Human brain mapping in dystonia reveals both endophenotypic traits and adaptive reorganization
    • The authors present an MEG study in which they localise the responses evoked in the primary somatosensory cortex by electrical stimulation of different fingers. The impressive result is that the disturbance in sensory mapping is greatest in the hemisphere contralateral to the unaffected arm in patients with unilateral hand dystonia. Thus, there is a subclinical disorganisation of sensory cortex even in parts of the body where there are no overt motor symptoms.

    • Impaired sequence learning in carriers of the DYT1 dystonia mutation
    • This study reveals that carriers of the DYT1 gene mutation who have no obvious clinical symptoms nevertheless have subtle abnormalities in control of movement that may be related to the abnormalities observed in functional imaging and neurophysiological studies. This is further evidence to show that the genetic deficit is not suppressed in these individuals. They lack apparent symptoms either because they do not have an additional deficit that is required to express symptoms fully, or because their genetic deficit is expressed to a lesser extent than that of clinically affected patients.
