This study compared a multifactorial strategy that included stepwise implementation of behavioral modification and pharmacologic therapy targeted at hyperglycemia, hypertension, dyslipidemia, and microalbuminuria, along with secondary prevention of cardiovascular disease with conventional aspirin treatment in patients with type 2 diabetes and microalbuminuria. The multifactorial intensified intervention reduced the incidence of cardiovascular and microvascular events by about 50%.

The protective effect was associated with the preservation of pancreatic beta-cell function and appeared to be mediated by a reduction in the secretory demands placed on beta-cells by chronic insulin resistance.
