PAR-2, a GPCR for trypsin, levels of which are elevated in IBD, is highly expressed in the intestine. Colonic administration of PAR-2 agonists to mice increased PAR-2 expression, caused inflammation, increased colonic permeability, and induced bacterial translocation, suggesting that PAR-2 may be involved in IBD pathogenesis.

Using DNA microarray analysis and immunohistochemistry, the authors show that isolated murine colonic epithelial cells express reduced levels of detoxification molecules involved in inhibiting inflammation in chronic, but not acute, colitis. In contrast, molecules with antiinflammatory functions are upregulated during both acute and chronic colitis.

Stable transfection of CARD15/NOD2 in Caco-2 cells decreased S. typhimurium survival, while expression of the NOD2 mutation associated with Crohn's disease was of no benefit.

The finding that the mdr1a (P-glycoprotein-encoding gene) knockout mouse develops colitis similar to UC prompted this study. Allele frequencies and genotype distributions of the C3435T single nucleotide polymorphism indicate a higher frequency in UC patients than controls, indicating that an impairment in P-glycoprotein function may increase susceptibility to UC.

The authors suggest this agent may be of therapeutic value in restoring barrier function in intestinal diseases.
