Longitudinal assessment of premorbid cognitive functioning in patients with schizophrenia through examination of standardized scholastic test performance
-
Fuller R, Nopoulos P, Arndt S, et al. Longitudinal assessment of premorbid cognitive functioning in patients with schizophrenia through examination of standardized scholastic test performance. Am J Psychiatry 2002; 159:1183-1189. This paper evaluates premorbid cognitive functions in individuals who later develop schizophrenia. The data suggest that not only are achievement scores lower in pre-schizophrenia patients, but that these scores drop rather precipitously from grades 8 to 11, a time during which synaptic pruning occurs.

Neuropsychological functioning in elderly patients with schizophrenia and Alzheimer's disease
-
McBride T, Moberg PJ, Arnold SE, et al. Neuropsychological functioning in elderly patients with schizophrenia and Alzheimer's disease. Schizophrenia Res 2002; 55:217-227. This paper demonstrates the degree of cognitive impairment in schizophrenia patients compared with Alzheimer's patients. It is surprising that a measure of object naming was performed worse in the schizophrenia group. One might speculate that the left hemisphere may be selectively damaged in chronically ill schizophrenia patients.

This study is a double-blind study with relatively large sample sizes evaluating cognitive improvement in patients assigned to these commonly used neuroleptic agents. Patients taking risperidone and olanzapine were superior to the haloperidol group on a global cognitive scale. Risperidone was significantly better on measures of memory than other neuroleptic agents.
