The first study assessing the respiratory frequency response to changes in assist-control settings in terms of the duration of the ventilator inspiratory time. The alterations in ventilator inspiratory time were achieved by changing tidal volume, flow, and inspiratory pause. The investigators concluded that the imposed ventilator inspiratory time during mechanical ventilation can determine respiratory frequency independently of delivered inspiratory flow and tidal volume.

The first study demonstrating that strategies to reduce the ventilator inflation time in patients with COPD, while causing tachypnea, prolong the time for exhalation and, consequently, decrease PEEPs. The investigators hypothesize that this prolongation in the time for exhalation results from the overdistension of some lung units during early inflation because of time-constant inhomogeneities in the lungs of patients with COPD. Early overdistension during early inflation would, in turn, cause neural inspiratory time to be shorter than in healthy subjects.

Indirect measurements of the duration of inhalation and exhalation were taken in patients with acute lung injury. Neural exhalation increased with increasing tidal volume and pressure support ventilation, whereas it remained constant with flow changes. Neural inhalation increased with decreasing flow but was not affected by tidal volume changes. Breathing frequency increased with decreasing tidal volume and pressure support and with increasing flow. The investigators conclude that alterations in commonly used ventilator settings induce acute changes in respiratory timing, probably through mechanoreceptor reflex feedback.

The duration of neural inhalation and neural exhalation during assist-control ventilation was recorded in 15 healthy subjects. The duration of neural inhalation decreased progressively as a function of the ventilator's inspiratory flow. Changes in neural exhalation were inconsistent and not significant. Moreover, the investigators determined that increases in flow decreased the ratio of duration of inhalation over the duration of the respiratory cycle, an important determinant of inspiratory muscle energetics.

In this study conducted in healthy subjects during non-rapid eye movement sleep, normocapnic assist-control ventilation with large tidal volumes decreased the amplitude of respiratory motor output. Hypocapnia did not further inhibit respiratory motor output amplitude beyond the effect of normocapnic assist-control ventilation at high tidal volumes but did cause a highly variable prolongation of time of exhalation when the end-tidal carbon dioxide was reduced by more than 3 mm Hg. According to the investigators, these data support the existence of a significant nonchemical inhibitory influence on the amplitude of respiratory motor output during assist-control mechanical ventilation at increased tidal volume.
