Using a non-human primate model of transient focal ischemia, the authors show that E-selectin immunoreactivity is present within the ischemic basal ganglia by 2 hours after reperfusion (following 3 hours of middle cerebral artery occlusion). The amount of E-selectin in the ischemic basal ganglia increased at 24 hours at which time it could also be identified within the non-ischemic basal ganglia.

Capitalizing on the release of central nervous system antigens into the circulation following ischemic breakdown of the blood-brain barrier, they were able to induce a relatively antigen and organ specific alteration in the immune response which resulted in a decrease in infarct size.

Using a transgenic mouse with a dominant negative mutation of the interleukin-1β converting enzyme, the authors show that stroke size and cerebral edema were decreased following 3 hours of ischemia and 24 hours of reperfusion. This was associated with a 77% decrease in IL-β production at 30 minutes after reperfusion and a decrease in DNA fragmentation at 24 hours in the mutant mice.

Whether or not TNF-α contributes to ischemic brain injury and how it does so has been debated. In this paper the authors examine the role of TNF-α in cerebral ischemia in vivo. Using several different strategies they show that TNF-α is clearly detrimental to the ischemic brain. Furthermore, they provide data from cell culture that show that TNF-α does not appear to directly affect neuronal function of survivial, which suggests a role for glial cells or inflammatory cells in the brain injury that occurs in vivo.

Lancet 1996; 347:422-425. In a prospective and consecutive study, the authors evaluated 390 stroke patients admitted to the hospital within 6 hours of stroke onset. They found that patients with mild hypothermia upon admission had a decrease in morbidity (as measured by the Scandinavian Stroke Scale) and mortality while those patients with hyperthermia had an increase in morbidity and mortality. For each degree (centigrade) rise in temperature the relative risk of poor outcome rose by 2.2. While this study clearly shows an association between fever and outcome following stroke, it does not address causality.

Furthermore, aspirin was only beneficial in preventing myocardial infarction in those men with the highest levels of C-reactive protein, suggesting that aspirin was acting as an anti-inflammatory agent as opposed to an anti-platelet agent.
