Although deficits in cortical gray matter volume were present in multiple cortical areas, this paper also replicates the finding of an anterior to posterior gradient in the magnitude of the deficits, with frontal and temporal regions showing greater deficits than parietal and occipital areas.

The magnitude of the deficits in cortical gray matter volume reported in this study are much smaller than those reported in samples of patients with chronic schizophrenia. It remains to be determined whether this can be explained by some degree of progression of the brain abnormalities in the early phase of the illness or is more likely the result of a selection bias.

This MRI study involves a unique sample of 57 patients hospitalized in a California state hospital, including 21 patients whose onset of psychosis was between the ages of 7 and 14 years. Although the group as a whole had striking deficits in gray matter volumes, no association was found between the magnitude of the deficits and either the age of onset (range 7-29 years) or the duration of illness (range 4-40 years). This study provides no evidence for progression of brain abnormalities over the long-term course of the illness, but it does suggest that more severely affected individuals may have brain abnormalities of greater magnitude.

This MRI study found significant intracranial volume reduction only in frontal lobes but evidence of relative tissue loss in all major regions assessed. This article includes a particularly thoughtful discussion dealing with the question of whether the MRI findings in schizophrenia can be best explained as the result of a single process or multiple processes.

Patients with schizophrenia were shown to have deficits in gray matter volumes in multiple brain regions relative both to control individuals and to patients with bipolar disorder. This finding is consistent with previous studies and suggests that changes in the cortex might provide clues to the neuropathology that underlies schizophrenia.

Eur Neuropsychopharmacol 1997; 7:25-38. The S-isomer but not the R-isomer of ketamine was associated with frontal and thalamic activation as well as induction of psychopathology. The comparison of isomers shows itself to be a very compelling method for delineating causal relationships.

Psilocybin was used to induce psychotic symptoms and was associated with metabolic hyperfrontality. The same group has reported metabolic hyperfrontality with ketamine-induced psychosis. Converging evidence from different challenges that induce broadly similar psychopathology helps focus on the metabolic changes that might be more central to the manifestation of psychosis.

Endogenous dopamine displaces the ligand iodobenzamide from its binding to dopamine receptors. The amphetamine-induced displacement of iodobenzamide in patients with schizophrenia was statistically higher than in control individuals suggesting that patients may have a dysregulation of presynaptic dopamine release. This ability to assess endogenous transmitter, and its particular application in concert with a pharmacological challenge, is likely to become an important new functional imaging paradigm.
