Virology 1997; 230:244-251. Comparison of nucleotide sequences from the E/NS1 region of the dengue virus genome allows the evolutionary relationships of dengue virus strains to be determined. Using this approach, this paper shows that the introduction of southeast Asian strains of dengue 2 into the western hemisphere is associated with the appearance of DHF/DSS in this region. Understanding the basis for the variations in virulence between different strains of dengue virus will be important for devising vaccine strategies targeted at the prevention of DHF/DSS.

In a series of experiments, this paper shows that dengue virus binds selectively to highly sulphated heparan sulphate on target cells via E. Dengue virus serotype 2 was used throughout, but given the sequence homology within the putative binding domain of E, it is likely that this is true for all four serotypes of dengue virus. The elucidation of at least one component of the cellular receptor for dengue is a highly significant step towards a better understanding of the tropism and pathogenesis of dengue virus infection.

This paper provides strong evidence that NS1 plays a role in early RNA replication, and in doing so demonstrates the power of a method of transcomplementation of defective yellow fever virus using non-cytopathic Sindbis replicons. This technology has enormous potential for future studies of flaviviruses (and other RNA viruses) because it allows viruses with lethal mutations to be propagated so that the precise phenotypic effects of a mutation can then be determined.

It suggests that TNF-α may be an important mediator of endothelial cell activation during dengue virus infection. The use of similar model systems may yield further interesting clues to the pathogenesis of DHF/DSS, which is especially significant in the absence of an animal model of DHF/DSS.

Complete sequencing of the genomes of the parental dengue 2 virus 16681 strain and two genetic variants within the PDK-53 attenuated derivative revealed only nine nucleotide differences out of the nearly 11 kb long genome. This low level of nucleotide substitution, coupled with the preparation of infectious clones for each strain, will enable a fairly straightforward site-directed mutagenesis study to identify those changes responsible for the attenuation phenotype. Successful identification of these attenuating mutations should then provide the basis for genetically engineering live attenuated vaccines.

One of the intriguing observations made in this paper is the apparently efficient cleavage of the C/prM junction of the expressed product. It is this cleavage which leads to the correct maturation of the structural proteins as immunogenic VLPs. Expression of gene cassettes comprising the structural proteins alone have previously met with failure as a consequence of a requirement for viral protease activity supplied by NS2b/3. Presumably no such restrictions occur in yeast cells?

Immunization with plasmids expressing the dengue-2 virus prM and E proteins were shown to elicit neutralizing antibody in mice, although only after at least three intradermal inoculations. The relatively slow antibody response reported in this paper (not dissimilar to reports of DNA immunization in other systems) highlights a potential problem for future experiments examining the protective efficacy of DNA vaccines for dengue. Dengue virus challenge in mice needs to be performed by the intracranial route and mice older than 6-10 weeks are less susceptible to infection. The mouse model may be confined to studies of immunogenicity with protection subsequently examined in primates.

There are very few carefully designed trials of treatment of DHF/DSS. The importance of these is emphasized by the conclusion from this study that AC-17 is not a valuable treatment for DHF/DSS, in contrast to the earlier reports of beneficial effects in less well-designed studies.
