This thorough clinical investigation examines the hypothesis that individual variations in systemic ifosfamide metabolism are associated with a higher risk of nephropathy. Results show a negative correlation between production of dechloroethylated ifosfamide metabolites and renal injury. The authors speculate that differences in renal rather than hepatic drug metabolism may be responsible for nephrotoxicity.

This study convincingly demonstrates that inhibition of γ-glutamyl transpeptidase prevents cisplatin nephrotoxicity without altering kidney platinum uptake or content. It is an excellent example of potential glutathione conjugate-mediated toxicity and of the importance of considering renal metabolism when evaluating drug-induced nephrotoxicity.
