Several studies over the past decade have shown that the membrane-attack complex of complement is deposited in small muscle blood vessels in DM, especially juvenile DM but also adult DM. This is felt to be a major mechanism of muscle injury in DM. This study demonstrates that this complex may also be involved in the cutaneous lesions of DM.

A description of these antibodies is of great interest, but they are not yet able to provide diagnostic information beyond demonstration of autoimmunity.

The clinical picture of myositis was examined in 30 patients from a population that had not previously been studied. Two patients were found to have autoantibodies to nuclear pore complexes, but surprisingly, none had anti-Jo-1. The 10-year survival rate was 85%.

Arthritis Rheum 1996, 39:868-876. Although the association of anti-Mi-2 with HLA-DR7 is known, this study examined the immunogenetics of anti-Mi-2 in greater detail. Thirty-one percent of the patients with anti-Mi-2 were homozygous for DR7, 100% had a tryptophan at position 9 of DRβ, with 81% homozygous for this finding. Of additional interest is the detailed examination of the clinical findings in the anti-Mi-2 subgroup of DM.

This study focused on the fact that some antisynthetase patients do not show clinical signs of myositis. All patients had a picture otherwise compatible with the antisynthetase syndrome. Anti-L-12 was the most common antisynthetase in this group.
