Classification criteria for polymyositis and dermatomyositis

A multicenter collaborative study of a new criteria set raises the issue of using clinical signs of systemic inflammatory illness and muscle tenderness as diagnostic criteria in IIMs. This study illustrates the importance of the comparison group in assessing the sensitivity and specificity of criteria.

The role of microvascular injury in the pathogenesis of cutaneous lesions of dermatomyositis
-
Crowson AN, Magro CM: The role of microvascular injury in the pathogenesis of cutaneous lesions of dermatomyositis. Hum Pathol 1996, 27:15-19. Several studies over the past decade have shown that the membrane-attack complex of complement is deposited in small muscle blood vessels in DM, especially juvenile DM but also adult DM. This is felt to be a major mechanism of muscle injury in DM. This study demonstrates that this complex may also be involved in the cutaneous lesions of DM.

Seelig HP, Moosbrugger IU, Ehrfeld H, Fink T, Renz M, Genth E: The major dermatomyositis-specific Mi-2 autoantigen is a presumed helicase involved in transcriptional activation. Arthritis Rheum 1995, 38:1389-1399. Excellent study that combines the molecular analysis of the Mi-2 antigen (later found to be one of at least two forms of Mi-2, this one labeled Mi-2β) with the use of a recombinant fragment of the molecule to examine the frequency and disease specificity of the autoantibody in a large population of patients.

J Exp Med 1996, 184:1313-1318. Sixty-two percent of myositis and 58% of lupus patients were shown to have anti-proteasome antibodies. Arthritis Rheum 1996, 39:868-876. Although the association of anti-Mi-2 with HLA-DR7 is known, this study examined the immunogenetics of anti-Mi-2 in greater detail. Thirty-one percent of the patients with anti-Mi-2 were homozygous for DR7, 100% had a tryptophan at position 9 of DRβ, with 81% homozygous for this finding. Of additional interest is the detailed examination of the clinical findings in the anti-Mi-2 subgroup of DM.

Interstitial lung disease with autoantibodies, against aminoacyl-tRNA synthetases in the absence of clinically apparent myositis

Friedman AW, Targoff IN, Arnett FC: Interstitial lung disease with autoantibodies, against aminoacyl-tRNA synthetases in the absence of clinically apparent myositis. Semin Arthritis Rheum 1996, 26:459-467. Antisynthetases were first described in PM/DM, but ILD is also very common in such patients and is often a more significant clinical problem. This study focused on the fact that some antisynthetase patients do not show clinical signs of myositis. All patients had a picture otherwise compatible with the antisynthetase syndrome. Anti-L-12 was the most common antisynthetase in this group.
