Br J Cancer 1996, 73:1556-1559. In this study the clinical growth kinetics of lung metastases are related to proliferation kinetics obained by flow cytometry on tissue obtained from the primary tumor. These results show an SPFR similar to those reported by others. It highlights the importance of the cell loss factor in the final assessment of clinical growth.

In 155 STSs, the SPF was measured with flow cytometry. The median SPF was 6.8%, with higher values in nondiploid tumors. Only in diploid tumors (55.5% of this series) was SPF a prognostic factor for survival.

Histopathology 1996, 28:437-444. In 23 STSs, the proliferative activity was scored with MIB-1 (Ki-67) and staining for iododeoxyuridine. Both proliferation indices were positively correlated (r = 0.78), but none was correlated with p53 positivity or bcl-2 expression.
13. Golouh R, Bracko M. Novak J: Predictive value of proliferation markers, • P53, and DNA ploidy for survival in patients with soft tissue spindlecell sarcomas. Mod Pathology 1996, 9:919-924. In this retrospective study of 60 spindle-cell sarcomas, no correlation was observed between p53 positivity and proliferation markers (PCNA or Ki-67). Previous reports had mentionned such a relationship in STSs.
14. Hirakawa N, Naka T, Yamamoto I, Fukuda T, Tsuneyoshi M: • Overexpression of bcl-2 protein in synovial sarcoma. Hum Pathol 1996, 27:1060-1065. Immunohistochemical staining for bcl-2 was performed in 48 STSs and was shown to be limited to synovial sarcomas. These authors state that bcl-2 positivity is even more restricted to the spindle cell component in biphasic synovial sarcomas. This study suggests a relationship between bcl-2 positivity and the X;18 translocation.

Sixteen of 47 grade I STSs expressed between 0.25 and 46.2 ng p53 protein per milligram of protein. Seven expressed 4.5 ng/mg or less, and nine expressed more than 4.5 ng/mg. After a mean follow-up of 112 months. 63% of patients with p53-positive tumors, but only 16% of p53-negative tumors, had died. Patients with tumors with more than 4.5 ng/mg mutant p53 had an even worse survival.

Of 145 patients with STSs, p53 mutations were identified in 15. This analysis shows the importance of nonframeshift mutations as a predictor for poor survival.

In this study, 16 of the 113 different sarcomas exhibited p53 mutations on DNA analysis. p53 Mdutations were more common in osteosarcomas and liposarcomas compared with MFH. In 69 cases, immunohistochemistry was compared with the DNA analysis. The results were concordant in 64.

Int J Cancer 1996, 69:268-272. The expression of the uPA was quantified in frozen tumor tissue from 69 STSs. The median level was 1.4 ng/mg protein (0.04 to 10.6). Leiomyosarcomas and MFHs had the highest median uPA levels. Increasing uPA levels correlated with increasing tumor grade, tumor necrosis, local recurrence, and metastasis. This is the first study on uPA levels in STSs, and it suggests that increased protease levels reflect the malignant phenotype.

Hurskainen T, Soini Y, Tuuttila A, Höyhtlyä M, Oikarinen A, Autio-• Harmainen H: Expression of the tissue metalloproteinase inhibitors TIMP-1 and TIMP-2 in malignant fibrous histiocytomas and dermatofibromas as studied by in situ hybridisation and immunohistochemistry. Hum Pathol 1996, 27:42-49. The expression of tissue inhibitors of metalloproteinases in both benign soft tissue tumors and STSs is reported. The investigators demonstrate an increased TIMP expression in sarcomas as opposed to in dermatofibromas. This observation is in accordance with those made in epithelial cancers, suggesting the absence of a clear correlation between metalloproteinase/inhibitor ratio and the invasive phenotype. In situ hybridization for TIMP-1 and TIMP-2 mRNA showed that the tumor cells produced these inhibitors of matrix-degradating enzymes.

This might be caused by an increased expression of hypoxia-inducible genes, such as those encoding for angiogenic factors. It is tempting to speculate that those tumors with the most pronounced hypoxia have the most intense neovascularisation and thus the largest window of opportunity for metastasis.

Cancer Res 1996, 56:5347-5350. In patients with localized high-grade sarcomas, the improvement of tumor hypoxia after hyperthermia and radiotherapy predicted for disease-free survival.
29. 31P magnetic spectroscopy spectra accurately localized 20 STSs and showed a prominent triphosphate nucleotide pool suggestive of a large fraction of metabolically viable cells. However, these studies also showed important intratumor and intertumor variations in accordance with histological variations.

A prospectively collected, single-institution, database of 1041 patients with localized STS is analyzed. Although local recurrence per se increased the risk for distant recurrence, prognostic factors for local and distant recurrence were clearly different, shedding doubt on the presumed intricate relationship between local and distal relapse in STSs.

A series of 216 patients with MFH is reported with an in-depth analysis of prognostic factors, again confirming tumor grade as the single most important predictor for disease-specific survival.

Am J Surg Pathol 1996, 20:391-405. A detailed analysis of the Clinicopathological features in a series of 75 patients with this variant of MFH is reported. It is a preferential limb sarcoma occuring at older age and with a particular tendency for local recurrence.
38. Brady MS, Perino G, Tallini G, Russo P, Woodruff JM: Malignant • mesenchymoma. Cancer 1996, 77:467-473. Eight patients suffering from these very rare sarcomas are described. The authors emphasize the application of rigid pathological criteria before making this diagnosis. The malignant mesenchyma represents a highly aggressive STS.

J Bone Joint Surg 1996, 78-B:658-661. The importance of a planned primary radical excision is illustrated by the fact that after reexcision, residual tumor was found in 59% of cases.

This study confirms the need for a planned excision of STSs and even suggests reoperation in all cases in which the initial surgery was unplanned; ie, revealed an initially unexpected STS.
46. Heslin MJ, Woodruff J, Brennan MF: Prognostic significance of a posi• tive microscopic margin in high-risk extremity soft tissue sarcoma: Implications for management. J Clin Oncol 1996, 14:473-478. Of 168 high-risk extremity STSs, 42 had a positive microscopic margin. This was a significant negative prognostic factor. It is suggested that the positivity of the surgical margin might reveal a biological characteristic of the tumor rather than the mere quality of the surgical resection.

In an update on this randomized study, previous results are confirmed, in that adjuvant brachytherapy improves local control rate after complete excision, but only in patients suffering from high-grade STSs.

In this study, patients with high-grade STSs treated with combined external and interstitial irradiation after local excision fared better than those treated only with brachytherapy.
