In this study the clinical growth kinetics of lung metastases are related to proliferation kinetics obained by flow cytometry on tissue obtained from the primary tumor. These results show an SPFR similar to those reported by others. It highlights the importance of the cell loss factor in the final assessment of clinical growth.

Only in diploid tumors (55.5% of this series) was SPF a prognostic factor for survival.

Immunohistochemical staining for bcl-2 was performed in 48 STSs and was shown to be limited to synovial sarcomas. These authors state that bcl-2 positivity is even more restricted to the spindle cell component in biphasic synovial sarcomas. This study suggests a relationship between bcl-2 positivity and the X;18 translocation.

Sixteen of 47 grade I STSs expressed between 0.25 and 46.2 ng p53 protein per milligram of protein. Seven expressed 4.5 ng/mg or less, and nine expressed more than 4.5 ng/mg. After a mean follow-up of 112 months. 63% of patients with p53-positive tumors, but only 16% of p53-negative tumors, had died. Patients with tumors with more than 4.5 ng/mg mutant p53 had an even worse survival.

Int J Cancer 1996, 69:268-272. The expression of the uPA was quantified in frozen tumor tissue from 69 STSs. The median level was 1.4 ng/mg protein (0.04 to 10.6). Leiomyosarcomas and MFHs had the highest median uPA levels. Increasing uPA levels correlated with increasing tumor grade, tumor necrosis, local recurrence, and metastasis. This is the first study on uPA levels in STSs, and it suggests that increased protease levels reflect the malignant phenotype.

Hum Pathol 1996, 27:42-49. The expression of tissue inhibitors of metalloproteinases in both benign soft tissue tumors and STSs is reported. The investigators demonstrate an increased TIMP expression in sarcomas as opposed to in dermatofibromas. This observation is in accordance with those made in epithelial cancers, suggesting the absence of a clear correlation between metalloproteinase/inhibitor ratio and the invasive phenotype. In situ hybridization for TIMP-1 and TIMP-2 mRNA showed that the tumor cells produced these inhibitors of matrix-degradating enzymes.

Tumor oxygenation predicts the likelihood of distant metastases in human soft tissue sarcoma
-
2 being less than or greater than 10 mm Hg. This suggests that tumor hypoxia predits for early metastasis. This might be caused by an increased expression of hypoxia-inducible genes, such as those encoding for angiogenic factors. It is tempting to speculate that those tumors with the most pronounced hypoxia have the most intense neovascularisation and thus the largest window of opportunity for metastasis.

A prospectively collected, single-institution, database of 1041 patients with localized STS is analyzed. Although local recurrence per se increased the risk for distant recurrence, prognostic factors for local and distant recurrence were clearly different, shedding doubt on the presumed intricate relationship between local and distal relapse in STSs.
