Vickers M: Estimation of the number of mutations necessary to cause chronic myeloid leukaemia from epidemiological data. Br J Haematol 1996, 94:1-4. A thoughtful mathematical analysis of the exponential rise of the incidence of CML with age. Coupled with the observation that the chance of progression does not seem to depend on the length of the chronic phase, this analysis leads to the conclusion that there are three rate-limiting mutations involved in the generation of chronic phase CML, but only one further stem cell mutation necessary for progression to transformation and blast crisis.

At 3 years the survival was better in the interferon alfa plus cytarabine group than in the interferon-alfa alone group (85.7% versus 79.1%, P< 0.02).

The authors demonstrate that with their transplant regimen, transplants using marrow cells from unrelated donors with a high frequency of alloreactive cytotoxic T-cell precursors in their blood were more likely to result in severe GVHD than transplants using donors with low cytotoxic T-cell precursor frequencies. A subset of patients who were seropositive for CMV and whose donors had high cytotoxic T-cell precursor frequencies fared particularly badly.

9/L. In 14 patients the leukapheresis product was entirely Ph-negative, and in 4 others it was almost entirely Ph-negative. Twelve patients were autografted and six have Ph-negative marrows 7 to 15 months postautograft.
