J Clin Oncol 1997, 15:1767-1777. This International Bone Marrow Transplant Registry report presents data on over 2000 patients undergoing allogeneic SCT for acute leukemia and CML. Outcome was determined by stage of leukemia, donor-recipient relationship, and degree of HLA matching. For early leukemia, alternative donor transplants were more than twice as likely to fail compared to HLA-identical sibling allogeneic SCT.

Allogeneic SCT using HLA-mismatched family members has been associated with high morbidity and mortality. The South Carolina group has used intensive pretransplantation conditioning and T-cell depleted, mismatched family member grafts as treatment for high-risk acute leukemia and CML. Their results suggest that this is a feasible option in patients who lack an HLA-identical donor.

This approach may make transplantation a feasible option for larger numbers of patients with hematologic malignancies.

Science 1997, 276:1719-1724. This paper describes a novel approach to treating leukemic relapse following allogeneic SCT. A viral "suicide gene" is transduced into donor lymphocytes ex vivo; the lymphocytes are then infused into the relapsing patient to confer GVL effect. Those patients who develop severe GVHD are given ganciclovir, which eliminates the offending lymphocyte population.
