Proc Natl Acad Sci U S A 1995, 92:8803-8807. Because of the imprecision of HLA-Cw serology, the biologic role of HLA-Cw in transplantation has been overlooked. A DNA-based HLA-Cw typing technique (PCR restriction fragment length polymorphism) was developed that allows the typing of over 22 different HLA-Cw alleles and identifies a series of new HLA-Cw alleles. The molecular identification of previously unrecognized HLA-Cw alleles and new HLA-Cw alleles (undetected by serology) has led to a reevaluation of the impact of Cw matching in transplantation.

Comparison between two cohorts of bone marrow allograft recipients (39 matched unrelated donors vs 39 HLA-identical sibling donors) reveals no significant difference in terms of incidence of grade I acute GVHD (63% vs 57%), actuarial probability of grade II-IV GVHD (15% vs 11%), chronic GVHD (29% vs 22%), or 2-year survival (59% vs 78%).

A DNA-based HLA haplotype matching technique using non-HLA DNA polymorphisms in the MHC as markers of blocks of ancestral haplotypes identifies shared evolutionary related HLA haplotypes. Such haplotype matching appears beneficial in terms of lower incidence of GVHD and longer survival in a series of 45 BMT cases.

Based on the assumption that family donors are better donors because they share complete haplotypes with the recipient, this work suggests that a suitable donor should be sought among cousins and blood-related aunts and uncles (the extended family). This would reduce the search time. A computer program is proposed that calculates the probability of finding a donor in the extended family given the combination of haplotype frequencies and family composition.
