The SUPPORT Principal Investigators: A controlled trial to improve care for seriously ill hospitalized patients: the study to Understand Prognoses and Preferences for Outcomes and Risks of Treatment (SUPPORT). JAMA 1995,274:1591-1598. This large interventional study showed that even when physicians were given information about patient prognoses and preferences for care, outcomes of end-of-life care were unchanged. Fewer than half of the physicians studied knew when their patients preferred to avoid CPR; nearly half of all DNR orders were written 48 hours prior to death; nearly half of all patients died in moderate to severe pain; and almost half of all patients who died spent at least 10 days in an ICU.

The rule of double-effect states that what would be morally wrong if done intentionally is permissible if it is foreseen but unintended. Therefore, high doses of opioids given to manage a terminally ill patient's pain would be acceptable even if the medication caused the patient's death. This article addresses the religious and philosophic origins of this rule and its shortcomings in clinical practice.
