Arch Surg 1998; 133:94-100. Pentoxifylline is a phosphodiesterase inhibitor that reduces TNF-α gene transcription. In this preliminary clinical trial, pentoxifylline reduced the severity of organ dysfunction, but did not improve survival. A larger study to determine the effects on clinically relevant outcomes may be warranted.

This retrospective study proposed to examine the risk factors for positive central venous catheter cultures and the clinical consequences of a positive catheter culture. Blood cultures were positive with the same organism in 45% of catheters with > 15 colonies on semiquantitative culture. This study suggests that bacteremia secondary to central venous catheter infections is frequent and mortality is approximately 20%.

This study supports the authors' previous report that the establishment of an organized trauma system improves outcomes. In this case, improvements in trauma case fatality rates were compared between Oregon and Washington states, and a greater improvement was found in Oregon, consistent with a beneficial impact of trauma system establishment.

This improvement is potentially related to increased experience with trauma, and could not be attributed to any specific intervention.

This report suggests that survival outcome in the most severely injured pediatric trauma patients is worse at the higher volume trauma centers. The authors suggest that high volume puts excessive stress on available resources and compromises care and outcomes. Although the results must be carefully interpreted because there was little adjustment for injury severity in the analyses, the points raised are important. Trauma systems and centers must assure availability of resources to maximize survival.

    • Reduced mortality in association with the acute respiratory distress syndrome (ARDS)

A comparison between cohorts of patients with ARDS from consecutive time periods from 1990-1997 indicated that there was a significant reduction in mortality from 66% in the initial 3 years to 34% in the subsequent 4 years. The authors were unable to attribute this reduction in mortality to any changes in patient demographics or underlying causes of ARDS. However, multivariate analysis was not performed, and it may be that the decrease in mortality was due to a reduction in disease severity or case mix. Although the use of inverse ratio mechanical ventilation was increased in the second time period, the authors were careful not to attribute the reduction in mortality to this therapy. They concluded that the improvement in survival was likely due to general improvements in care over time.

The differences In study design between this study and the study reported by Amato et al. [23••] warrant careful review of each of these studies.

N Engl J Med 1998; 338:791-797. This important clinical trial suggests that ranitidine is more effective than sucralfate in preventing upper gastrointestinal bleeding. Despite the rigorous design, analysis and reporting, this study is limited by failing to document the actual site of bleeding in a majority of patients with bleeding. Ranitidine was no more effective when only patients with documented stress ulceration were included.

This study presents data from a clinical trial comparing ranitidine with sucralfate stress ulceration prophylaxis in trauma victims. There was an increase in total infectious complications with ranitidine use, suggesting a potential adverse systemic (immunologic) effect. These results contrast with previous reports that suggest a beneficial immunologic effect of ranitidine in post-operative patients or victims of traumatic brain injury.

This report documents a strong association between complications (eg, ARDS, pneumonia) and resource utilization, and suggests that prolonged length of stay can be attributed to complications. Mortality in patients with complications and prolonged length of stay was extremely low. It was suggested that complications and excessive length of stay could be used as important outcome measures after trauma.

The diagnosis was based on specimens obtained with bronchoscopy. A high incidence of methicillin-resistant Staphylococcus aureus was reported to be associated with previous antibiotic use.
