Interventions to reduce colonisation and transmission of antimicrobial-resistant bacteria in intensive care units: An interrupted time series study and cluster randomised trial
-
Derde LP, Cooper BS, Goossens H, et al. Interventions to reduce colonisation and transmission of antimicrobial-resistant bacteria in intensive care units: an interrupted time series study and cluster randomised trial. Lancet Infect Dis 2014;14:31-39. A European interrupted time series study of universal chlorhexidine body washing combined with hand hygiene improvement followed by a cluster-randomized trial with either conventional (culture) or rapid screening (PCR) for MRSA and VRE and contact precautions. In the context of a sustained high level of compliance to hand hygiene and chlorhexidine bathings, screening and isolation of carriers do not reduce acquisition rates of multidrug-resistant bacteria, whether or not screening is done with rapid testing or conventional testing.

-
Marshall C, Richards M, McBryde E. Do active surveillance and contact precautions reduce MRSA acquisition? A prospective interrupted time series. PLoS One 2013;8:e58112. A preplanned prospective interrupted time series comparing rapid PCR detection and use of long sleeved gowns and gloves (contact precautions), and single room isolation or cohorting of MRSA-colonized patients with a control group. Contact precautions with single room isolation or cohorting were associated with a 60% reduction in MRSA acquisition.

Universal glove and gown use and acquisition of antibiotic-resistant bacteria in the ICU: A randomized trial
-
Harris AD, Pineles L, Belton B, et al. Universal glove and gown use and acquisition of antibiotic-resistant bacteria in the ICU: a randomized trial. J Am Med Assoc 2013;310:1571-1580. A cluster-randomized trial assessing whether wearing gloves and gowns for all patient contact in the ICU decreases acquisition of MRSA or VRE compared with usual care. The use of gloves and gowns for all patient contact compared with usual care among patients in ICUs did not result in a difference in the primary outcome of acquisition of MRSA or VRE. There was a lower risk of MRSA acquisition alone and no difference in adverse events.

-
Climo MW, Yokoe DS, Warren DK, et al. Effect of daily chlorhexidine bathing on hospital-acquired infection. N Engl J Med 2013;368:533-542. A multicenter, cluster-randomized, nonblinded crossover trial to evaluate the effect of daily bathing with chlorhexidine-impregnated washcloths on the acquisition of MDROs and the incidence of hospital-acquired bloodstream infections. Daily bathing with chlorhexidine-impregnated washcloths significantly reduced the risks of acquisition of MDROs and development of hospital-acquired bloodstream infections.

Targeted versus universal decolonization to prevent ICU infection
-
Huang SS, Septimus E, Kleinman K, et al. Targeted versus universal decolonization to prevent ICU infection. N Engl J Med 2013;368:2255-2265. A pragmatic, cluster-randomized trial randomly assigned to one of three strategies, adult ICUs patients: MRSA screening and isolation; targeted decolonization (i.e. screening, isolation, and decolonization of MRSA carriers); universal decolonization (i.e. no screening and decolonization of all patients). Universal decolonization was more effective than targeted decolonization or screening and isolation in reducing rates of MRSA clinical isolates and bloodstream infection from any pathogen.
