12. Benazzi F, Akiskal HS. Refining the evaluation of bipolar II: beyond the strict SCID-CV guidelines for hypomania. J Affect Disord 2003; 73 (1-2):33-38. This is an interesting article on the issues in detecting and diagnosing bipolar II. The authors recommend that the diagnostic criteria for bipolar II hypomanic symptoms be shortened to less than 4 days based on a reevaluation of depressed patients who exhibited hypomanic symptoms using the Structured Clinical Interview for DSM-IV. The correct diagnosis of bipolar II is important as antidepressant monotherapy might induce rapid cycling (although others have suggested this is based on anecdotal evidence).

This systematic literature review highlights the research between 1985 and 2002 that focused on the quality of life, work productivity, use and costs of care effects of bipolar disorder.

Pharmacoeconomics. 2003; 21:1081-1090. This is one of several articles documenting the economic burden of bipolar disorder to come out this year. One of the key issues in conducting such studies, especially across the globe, is the concern regarding complete utilization and cost data. This article describes the limitations in relying on secondary claims data from France, and discusses how the costs of care for this illness may be underestimated due to the potential that this condition is underdiagnosed in France.

Evidence-based guidelines for treating bipolar disorder: recom. mendations from the British Association for Psychopharmacology. J Psychopharmacol 2003; 17:149-173. The British Association of Psychopharmacology guidelines published in April 2003 represent the state-of-the art in bipolar disorder pharmacotherapy, and provide a comprehensive summary of recommended acute and long-term (maintenance) therapy. These guidelines also pay particular attention to the issues surrounding 'off label' pharmacotherapy that do not have hard empirical evidence regarding treatment efficacy.
23. Goodwin GM, Young AH. The British Association for Psychopharmacology guidelines for treatment of bipolar disorder: a summary. J Psychopharmacol 2003; 17 (4 Suppl):3-6. This is a summary version of the Goodwin and Young article, which highlights advances in pharmacotherapy for the treatment of bipolar disorder. For example, they highlight current guidelines including the approval of atypical antipsychotics for the treatment of acute mania; and the use of lithium or carbamezepine for the treatment of mild to moderate mania.

The World Federation of Societies of Biological Psychiatry (WFSBP) Guidelines for the Biological Treatment of Bipolar Disorders, Part II: Treatment of Mania. World J Biol Psychiatry 2003; 4:5-13. Part I of the World Federation of Societies of Biological Psychiatry focuses on the treatment of mania. In treating mania, it is important to consider the type of mania (e.g., euphoric or classical mania, hypomania, psychotic mania) in order to tailor pharmacotherapy treatment strategies. These guidelines also highlight the importance of evaluating treatment tolerability.

J Psychiatr Res 2004; 38:285-294. This is an interesting article examining a new method for assessing provider adherence to guidelines for bipolar disorder treatment. Their key findings suggest that most providers were prescribing appropriate pharmacotherapy, but in some cases, many patients were not receiving an adequate dose.
29. Rollins G. Study questions bipolar disorder treatment guidelines. Rep Med Guidel Outcomes Res 2003; 14:7-9. This commentary questions the treatment guidelines for bipolar depression; notably, whether the discontinuation of antidepressants after symptom remission is appropriate. The article cites evidence that discontinuation of antidepressants can lead to increased relapses of depression in bipolar patients in most cases.
