This is an interesting article on the issues in detecting and diagnosing bipolar II. The authors recommend that the diagnostic criteria for bipolar II hypomanic symptoms be shortened to less than 4 days based on a reevaluation of depressed patients who exhibited hypomanic symptoms using the Structured Clinical Interview for DSM-IV. The correct diagnosis of bipolar II is important as antidepressant monotherapy might induce rapid cycling (although others have suggested this is based on anecdotal evidence).

One of the key issues in conducting such studies, especially across the globe, is the concern regarding complete utilization and cost data. This article describes the limitations in relying on secondary claims data from France, and discusses how the costs of care for this illness may be underestimated due to the potential that this condition is underdiagnosed in France.
