This study reports the long-term experience of a single national center. It is unique as it is the only large center in Belgium and that patients with CHD at this center are followed from 'fetus to adults'. It, therefore, likely accurately reflects changes in survival of patients born with CHD.

6. Khairy P, Ionescu-Ittu R, Mackie AS, et al. Changing mortality in congenital heart disease. J Am Coll Cardiol 2010; 56:1149-1157. This population-based study from Quebec provides unique data of changing patterns of mortality in CHD. It demonstrates that mortality has shifted almost entirely to adult life. It is, however, important to underscore that excess mortality of adult survivors with complex CHD is not reflected by this study, as these populations are just emerging.
7. Verheugt CL, Uiterwaal CS, van der Velde ET, et al. Mortality in adult congenital heart disease. Eur Heart J 2010; 31:1220-1229. The Dutch CONCOR national registry is the largest national registry of adults with CHD in Europe. From this database, several important questions around adults with CHD such as mortality, hospital admissions, and endocarditis among others have been addressed.

The Toronto Congenital Cardiac Centre for Adults has started to empirically investigate patients with CHD at the end of their lives. As a result of research, palliative care specialists have been integrated in clinical practice.

The Canadian Heart Health Strategy and Action Plan includes a model for chronic disease management and EOL planning and care. The authors give an overview of the problems such a strategy may be facing. Some of the suggested approaches amount to cultural changes in medical practice.
