Analysis of the NHANES data was reported with regards to changes in the prevalence of extreme obesity (using the definition of BMI-99% or-40 kg/ m2). Overall, the prevalence had increased by over 300% since 1976 and 70% between 1994 and 2004. The patterns of severe obesity were disparate with white children having minimal increases during 1988-2004, in contrast to a rise to more than 5% among Mexican American and Black children during the same period.

The Early Childhood Longitudinal Study measures heights and weights in preschool children in a nationally representative sample of US children. The 2005 data were analyzed for the five major US racial/ethnic groups (non-Hispanic white, Hispanic, non-Hispanic black, Asian and American Indian/Native Alaskan) to estimate the prevalence of obesity among 4-year-old children. There was an 18.4% overall prevalence found with obesity twice as common in the American Indian/Native Alaskan children (31.2%) as in non-Hispanic white (15.9%) or Asian children (12.8%). Hispanic children had a 22% and non-Hispanic black children had a 20.8% risk of obesity at age 4 years. The weight measurements were done in light clothing (in contrast to NHANES paper gowns) and focused on 4-year-olds in contrast to the 2-5-year comparable group in NHANES.

The Bogalusa Heart Study has measured height and weight on 11 653 children in 8 cross-sectional surveys. The proportion of children aged 5-17 years who are overweight/obese has risen from 14.2 to 48.4% and the proportion of obese children has increased from 5.6 to 30.8% between 1973 and 2009. This is a temporal trend with increases in all race-sex groups, suggesting that certain communities have high risk social and physical environments leading to obesity.

BMI data for children aged 6-18 years as well as adults from Australia, China, the UK and the USA were collected to measure trends form BMI 95% or more. Comparison of 6-year-old children BMI averages at the 95% were approximately 20-2 since 1986 with the exception of China. The Chinese average BMI 95% or more rose from 19.8 to 24.8 between 1997 and 2006 among sampled 6-year-old children.
