In the primary form, the initial allergic reaction occurs in the conjunctiva, whereas the secondary form is induced by the allergic reaction originating primarily in the nasal mucosa. The nasal allergen challenge combined with recording of conjunctival and subjective symptoms can distingish between the two forms.

NPTs with allergens performed by rhinomanometry in combination with recording of ocular signs and subjective symptoms are the only method that can confirm the participation of nasal allergy in AC and differentiate between the primary and secondarily induced forms of allergic keratoconjunctivitis.
