In this prospective, randomized, double-blinded, placebo-controlled trial, alteplase was administered in the 3-6h window to 101 patients with perfusion-diffusion mismatch. Alteplase was nonsignificantly associated with lower infarct growth and significantly associated with increased reperfusion in patients who had mismatch.
