Presents a multimodal protocol for management of post-TKA and total-hipreplacement analgesia that emphasizes the use of continuous PNBs. Compared with epidural and patient-controlled administered morphine treatment groups, the Total Joint Regional Pathway resulted in decreased narcotic requirement, nausea/vomiting, urinary retention, and sedation. Patients receiving regional analgesia were also more likely to transfer from bed to chair on the day of surgery and, overall, had a mean hospital stay of 2.8 days compared with 5 days for controls.

The effect of single-injection femoral nerve block versus continuous femoral nerve block after total knee arthroplasty on hospital length of stay and long-term functional recovery within an established clinical pathway. Anesth Analg 2006; 102:1234-1239. This study comparing single-shot and continuous femoral nerve blocks after TKA found that mean VAS scores and oxycodone consumption were significantly lower at all postoperative time points in patients with continuous PNB. Hospital length of stay and rehabilitation indices, however, were similar. Patient satisfaction was not assessed.
