This key article describes a large prospective case-control study during the Quebec outbreak which identified risk factors including fluoroquinolone use for the development of CDAD. The authors also performed strain typing and evaluated antibiotic susceptibility in a subgroup of their isolates.

Lancet 2005; 366:1079-1084. This important paper quantitatively compared toxin production of NAP1/027 isolates from Quebec to historic C. difficile isolates. They found markedly increased toxin production by the NAP1/027 strain in vitro, providing a putative mechanism of increased pathogenicity of this epidemic strain.
