Combined postmenopausal hormone therapy and cardiovascular disease: Toward resolving the discrepancy between observational studies and the Women's Health Initiative clinical trial

Prentice RL, Langer R, Stefanick ML, et al. Combined postmenopausal hormone therapy and cardiovascular disease: toward resolving the discrepancy between observational studies and the Women's Health Initiative clinical trial. Am J Epidemiol 2005; 162:404-414. This study directly compared findings from observational epidemiology and an RCT. Its strength lies in the fact that similar protocols were used in both the observational study and the RCT, with case ascertainment identical in both.

-
Hendrix SL, Wassertheil-Smoller S, Johnson KC, et al. Effects of conjugated equine estrogen on stroke in the Women's Health Initiative. Circulation 2006; 113:2425-2434. In this, the most recent publication from the Women's Health Initiative (WHI) RCT, unopposed estrogen was found to increase the risk of total stroke and ischaemic stroke. There was evidence of a protective effect on haemorrhagic stroke, but because of the small numbers of haemorrhagic stroke this was imprecisely estimated.

Eur Heart J 2006; 27:746-755. This primary prevention RCT examined the effects of tibolone (a selective oestrogen receptor modulator) or combined oestrogen/progestin both compared to placebo on the progression of carotid intima media thickness. Intima media thickness progression was greater in both intervention arms than in placebo.

Hormone replacement therapy and acute myocardial infarction: A large observational study exploring the influence of age
-
Kim J, Evans S, Smeeth L, Pocock S. Hormone replacement therapy and acute myocardial infarction: a large observational study exploring the influence of age. Int J Epidemiol 2006; 35:731-738. This prospective cohort study examined the hypothesis that age at initiation modifies the effect of HRT on CHD. The study found that any protective effect appeared greater at older age of initiation (opposite of what the hypothesis suggests).
