The evidence from these RCTs was evaluated to determine the impact of OPCAB on 12 outcomes. Favoring OPCAB: less severe inflammatory response, decreased perioperative blood loss, fewer blood transfusions, better myocardial function, lower incidence of atrial fibrillation, shorter duration of ventilation, fewer gastrointestinal complications, less expensive. No difference in patency rate, acute renal failure in low risk patients, quality of life, and cerebral dysfunction. The comparable incidence of stroke in the OPCAB group may be attributable to aortic manipulations (side-biting clamp).

The authors stressed that the previous RCTs remained underpowered to detect clinically important treatment effects. They recommended for future RCTs enrollment of higher-risk patients, blinding of outcome assessors, and assessment of longer-term outcomes.

This is a review of RCTs, nonrandomized and retrospective studies plus an analysis from two large databases (Society of Thoracic Surgeons and NY State). The two techniques have similar mortality in multi-vessel coronary artery disease, equivalent long-term patency, and freedom from surgical reintervention. There is a decrease in stroke rate favoring OPCAB. Elderly with comorbid risk factors and advanced atherosclerotic disease of the ascending aorta benefit most from OPCAB.
