The evidence from these RCTs was evaluated to determine the impact of OPCAB on 12 outcomes. Favoring OPCAB: less severe inflammatory response, decreased perioperative blood loss, fewer blood transfusions, better myocardial function, lower incidence of atrial fibrillation, shorter duration of ventilation, fewer gastrointestinal complications, less expensive. No difference in patency rate, acute renal failure in low risk patients, quality of life, and cerebral dysfunction. The comparable incidence of stroke in the OPCAB group may be attributable to aortic manipulations (side-biting clamp).

Circulation 2005; 111:2858-2864. This AHA scientific statement points out the contemporary literature on OPCAB versus CABG. They recommended a large scale prospective study to answer whether either strategy is superior, in which patient population. Until then, neither procedure should be judged inferior to the other.

19. Wijeysundera DN, Beattie S, Djaiani G, et al. Off-pump coronary artery surgery for reducing mortality and morbidity: Meta-analysis of randomized and observational studies. J Am Coll Cardiol 2005; 46:872-882. This is a meta-analysis of 37 RCT and 22 observational studies. There were no statistically significant reductions in short-term mortality and morbidity. The authors stressed that the previous RCTs remained underpowered to detect clinically important treatment effects. They recommended for future RCTs enrollment of higher-risk patients, blinding of outcome assessors, and assessment of longer-term outcomes.
20. Salzberg SP, Adams DH, Filsoufi F. Coronary artery surgery: conventional coronary artery bypass grafting versus off-pump coronary artery bypass grafting. Curr Opin Cardiol 2005; 20:509-516. This is a review of RCTs, nonrandomized and retrospective studies plus an analysis from two large databases (Society of Thoracic Surgeons and NY State). The two techniques have similar mortality in multi-vessel coronary artery disease, equivalent long-term patency, and freedom from surgical reintervention. There is a decrease in stroke rate favoring OPCAB. Elderly with comorbid risk factors and advanced atherosclerotic disease of the ascending aorta benefit most from OPCAB.

This observational study demonstrated the importance of surgeon experience in OPCAB, with respect to the need for future revascularization.

This article depicts the interactions between coronary branches and grafts, whose arrangement plays a major role in flow distribution. The graft arrangement may lead to flow reversal and subsequent occlusion.
27. Pepper J. Controversies in off-pump coronary artery surgery. CM&R 2005; 3:27-33. This makes a revision of the literature with an emphasis on the controversial aspects of off-pump surgery. The author concludes that for high-risk groups, OPCAB reduces the incidence of cerebral injury and the need for postoperative renal support.
