The authors describe thought-provoking observations on the overlap between GERD and dyspepsia. Most strikingly, epigastric pain was found to be associated with pathological oesophageal acid exposure.

Gastroenterology 2005; 128:532-540. The authors of this multicenter European trial are to be complimented for choosing a sham-controlled design, which is, regrettably but incontrovertibly, the only way in which the effect of surgical procedures on subjective endpoints can be assessed.

Helicobacter pylori eradication does not cause reflux oesophagitis in functional dyspeptic patients: a randomized, investigator-blinded, placebo-controlled trial. Aliment Pharmacol Ther 2005; 21:1231-1239. Some years ago concern was raised that H. pylori eradication might lead to exacerbation of GERD or to de-novo development of GERD. This study helps to take some of the fear away.
40. Fraser A, Delaney B, Moayyedi P. Symptom-based outcome measures for dyspepsia and GERD trials: a systematic review. Am J Gastroenterol 2005; 100:442-452. This review identified 26 disease-specific questionnaires for GERD, dyspepsia or both. Above all, this paper makes clear that there is a lack of consensus about which type of instrument is the most appropriate for clinical trials. The authors' recommendation that at least specific symptoms should be scored feels good, but whether this can be concluded from their review is debatable.
