This study is the first meta-analysis completed in the literature for proton spectroscopy and schizophrenia; this meta-analysis examines the results of NAA in various brain regions and compares NAA in first-episode and chronic patients. There is compelling evidence for NAA reduction in schizophrenia. Publication bias cannot be completely excluded.

Schizophr Res 2005; 73:209-219. While other studies have failed to show a relationship between NAA and disease duration, this study did find a significant inverse relationship in the left hemisphere. The relatively shorter disease duration in this sample may have permitted identification of the relationship between length of illness and NAA.

In this study, acute antagonism of NMDA receptors resulted in increased Gln in the anterior cingulate. These results were the first to document in humans that NMDA receptor blockade leads to a paradoxical increase in glutamatergic metabolism as documented in animals. An important missing piece supporting the NMDA receptor hypofunction model of schizophrenia is provided in this paper.

This model has the further advantage of using low dosage of PCP, unlikely to cause widespread neuronal loss. In schizophrenia, the post-mortem literature does not support widespread neuronal loss.

This is the first study to document increased NAA in the striatum of rats chronically treated with antipsychotic medication. It suggests that the increased striatal volumes with typical antipsychotics in humans are likely due to an increase in the density of neuronal processes. The absence of changes in NAA in cortical regions in antipsychotic-treated rats suggests that the NAA reductions in patients are not secondary to medication effects.
