    • Epidemiological characteristics and laboratory diagnosis of fungal keratitis: A three-year study
    • Bharathi MJ, Ramakrishnan R, Vasu S, et al.: Epidemiological characteristics and laboratory diagnosis of fungal keratitis: a three-year study. Indian J Ophthalmol 2003, 51:315-321. This retrospective study involved 3183 patients with corneal ulcer in a 3-year period from a single tertiary eye care center. All the data related to age, sex, risk factors, clinical features, and etiologic agents were carefully analyzed. Out of 3183 patients, 1095 (34.4%) had fungal keratitis. The Fusarium species was the principal pathogen (42.82%) Male patients were commonly affected (65%). Most of them (66.85%) were in the younger age group (21-50 years). Fungal keratitis patients had experienced ocular trauma (92%) and vegetable injury (61%). The sensitivity of KOH was 99%.

This author has published several articles on fungal keratitis and has vast knowledge of the laboratory diagnosis of microbial keratitis. This article highlights the sensitivity of various stains. Gram stain will identify fungal species in 45 to 73% of cases, and Giemsa stain will identify fungi in 66%. Lactophenol cotton blue has a sensitivity of 70 to 80%. The author suggests that the current treatment regimens for mycotic keratitis need to be modified.

There was no statistical difference in efficacy against any of the fungi encountered in the study: 57% was Fusarium species and 67% of cases were cured.

The corneas of 105 rabbits in seven groups were infected with viable yeast cells of Candida albicans to determine the most efficient time point and concentration of topical corticosteroids in Candida albicans keratitis treated with fluconazole. The authors concluded that treatment with fluconazole and adjunctive high-dose prednisolone is most effective when administered 9 days after infection, and therefore steroids are not contraindicated when given with antimycotic drugs in fungal keratitis. The issue is controversial and needs more data.

This article reviews the pharmacologic and pharmacokinetic properties and clinical usefulness of voriconazole. In head-to-head comparative trials, voriconazole appeared to be as efficacious as amphotericin B for the treatment of invasive aspergillosis. In clinical studies, it was as efficacious as fluconazole and useful against fluconazole-resistant and itraconazole- resistant strains of Candida. Dose adjustment is recommended in patients with hepatic dysfunction.

In this retrospective consecutive case series of 45 patients with severe fungal keratitis, grafting with cryopreserved cornea seemed to have yielded excellent results, in spite of the use of topical and oral fluconazole perioperatively, in the knowledge that fluconazole is supposed to be not effective against Fusarium and Aspergillus species.
