Cochrane Database Syst Rev 2005: CD004588. Review. This is an important systematic review of nine recent randomised clinical trials (3519 patients). Four trials included patients with multiple-vessel disease and five single-vessel disease. CABG was associated with fewer major adverse cardiac events (mainly repeat revascularisation). There was no difference in survival.

Circulation 2004; 109: 2290-2295. Surgeons from the Cleveland Clinic Foundation first documented the survival benefit of IMA grafting [4]. They are pioneers in bilateral IMA grafting [5]. This is a study of a cohort from the late 1990s including 872 patients treated with percutaneous stenting and 5161 CABG patients. This documents a survival benefit of surgery over PCI in a single-centre real-world patient population.

This powerful and large registry analysis from New York of almost 60 000 patients with multivessel disease documents a survival benefit after 3 years in favour of CABG. The power of the study lies in its large number of real-world patients and the multi-centre setting (35 hospitals).
