A large population-based study demonstrated a remarkable decline in cases of invasive pneumococcal disease throughout the US. This prospective controlled clinical trial showed a heptavalent pneumococcal polysaccharide-meningococcal outer membrane complex conjugate vaccine to have a 56% efficacy in protecting against episodes of AOM due to S. pneumoniae. Protection occurred after the second dose but did not extend to the non-vaccine serotypes contained in the serogroups found in the vaccine.

The proportion of non-vaccine serotypes causing infection increased substantially after licensure of the vaccine in children who had received at least two doses of vaccine. Capsular switching was documented.

All recipients received three doses of vaccine after transplantation; 77% of the recipients with vaccinated donors had protective IgG levels after the first dose compared with only 36% in those who did not receive vaccination before transplantation.

This study tries to evaluate not only the quantitative, but also the qualitative, response to an 11-valent conjugated pneumococcal vaccine by using an opsonophagocytic assay. It determines antibody concentrations required to kill pneumococci in vitro, but cannot be used to extrapolate protection against pneumococcal infection.
