This post-hoc analysis of the role of controlling glucose levels in critically ill surgical patients indicates that elevated glucose concentrations affect mitochondrial ultrastructure and function, giving further support for a key role of the liver in the development of metabolic collapse.

Soop M, Carlson GL, Hopkinson J, et al. Randomized clinical trial of the effects of immediate enteral nutrition on metabolic responses to major colorectal surgery in an enhanced recovery protocol. Br J Surg 2004; 91:1138-1145. This randomized trial of the effects of postoperative complete enteral nutrition after major lower gastrointestinal surgery shows that this treatment can be given while maintaining normal glucose levels without additional insulin. The plausible explanation for this is likely to be the avoiding of insulin resistance using preventive measures.
