Bim regulation may determine hippocampal vulnerability after injurious seizures and in temporal lobe epilepsy

Expression of the agonist Bcl-2-interacting mediator of cell death (Bim) is under the control of the forkhead in rhabdomyosarcoma (FKHR) transcription factors. The authors examined the response of this pathway to experimental seizures and in hippocampi from patients with intractable TLE. They showed that Bim levels were significantly lower than in controls and FKHR was inhibited. They were able to reproduce these results experimentally in rats by evoking multiple brief, noninjurious electroshock seizures. They concluded that Bim expression may be a critical determinant of whether seizures damage the brain, and that its control may be neuroprotective in status epilepticus and epilepsy.

Status epilepticus was induced in rats by electrical stimulation of the amygdala, and the progression of structural alterations was monitored with multiparametric MRI. Absolute T2, T1rho and diffusion images were acquired from amygdala, piriform cortex, thalamus and hippocampus for less than or 4.5 months after status epilepticus induction. Frequency and type of spontaneous seizures were monitored with video-electroencephalography recordings. Histological damage was assessed from Nissl, Timm and Fluoro-Jade B preparations at 8 months. There was no difference in the amount of neuronal loss and mossy fibre sprouting between the animals with rare and frequent seizures. The MRI detected tissue alterations during early epileptogenesis might not predict the severity of epilepsy.

No obvious neuronal loss was observed in the CA1 and CA3 sectors of the hippocampus, the amygdala, parahippocampal regions, or thalamus. A significant bilateral reduction in neuronal density was determined in the dentate hilus, but this did not significantly differ between rats with and without observed spontaneous recurrent seizures.
