Infliximab improves endothelial dysfunction in systemic vasculitis: A model of vascular inflammation
-
Booth AD, Jayne DR, Kharbanda RK, et al.: Infliximab improves endothelial dysfunction in systemic vasculitis: a model of vascular inflammation. Circulation 2004, 109:1718-1723. The authors used the most definitive test involving arterial infusions in a clinical physiology laboratory to confirm that ECD occurs in 1°SV, and they related this to disease activity. The improved EC function after therapy may well have related to the clinical remission achieved rather than to a specific effect of TNF blockade.

Inflammation and arterial stiffness in systemic vasculitis: A model of vascular inflammation
-
Booth AD, Wallace S, McEniery CM, et al.: Inflammation and arterial stiffness in systemic vasculitis: a model of vascular inflammation. Arthritis Rheum 2004, 50:581-588. This study demonstrated increased arterial stiffness that also correlated with disease activity, showing that the altered EC function is associated with clinically relevant changes in blood vessel function. This study and [6,7•] clearly establish the frequency and relevance of EC dysfunction in adult 1°SV.

Impaired endothelial function in the brachial artery after Kawasaki disease and the effects of intravenous administration of vitamin C
-
Deng YB, Li TL, Xiang HJ, et al.: Impaired endothelial function in the brachial artery after Kawasaki disease and the effects of intravenous administration of vitamin C. Pediatr Infect Dis J 2003, 22:34-39. This interesting study confirmed that the presence of ECD in childhood vasculitis and its persistence for as long as 10 years after the acute stage occurred irrespective of the type of initial therapy. Despite this persistence, the impairment was not fixed because it improved rapidly with ascorbate infusion. This suggests that the chronic maintenance of ECD is an active metabolic process that we are ignorant of at present but that urgently needs unraveling.

Altered myocardial flow reserve and endothelial function late after Kawasaki disease
-
Furuyama H, Odagawa Y, Katoh C, et al.: Altered myocardial flow reserve and endothelial function late after Kawasaki disease. J Pediatr 2003, 142:149-154. This study of Kawasaki disease at adult follow-up examined regional endothelial function in the coronary arteries directly, at catheterization, and correlated this with local myocardial flow reserve using a cold pressor test. Detailed analysis of multiple regions showed impairment according to both test results regardless of a history of aneurysm or current coronary anatomy. This firmly establishes a risk of late cardiovascular disease in all cases of Kawasaki disease.

Active but transient improvement of endothelial function in rheumatoid arthritis patients undergoing long-term treatment with anti-tumor necrosis factor alpha antibody
-
Gonzalez-Juanatey C, Testa A, Garcia-Castelo A, et al.: Active but transient improvement of endothelial function in rheumatoid arthritis patients undergoing long-term treatment with anti-tumor necrosis factor alpha antibody. Arthritis Rheum 2004, 51:447-450. The effects of TNF blockade on EC function are relevant both to understanding the mechanisms of EC dysfunction and to controlling it. This paper suggests a complex response variable with time, as was also noted in an earlier abstract on 1°SV (see [47]). The late loss of benefit apparent at day 28 seems to contradict the data of Booth et al. [7•], but the latter assessed 1°SV patients at 12 weeks, after remission had been obtained. Further detailed serial assessments are needed to differentiate between the direct effects of therapy on EC function and the indirect benefits of suppressing disease activity.

These cells actively secrete TNF-α and are most likely to be involved in the late clinical stage with plaque instability.

Selzer F, Sutton-Tyrrell K, Fitzgerald SG, et al.: Comparison of risk factors for vascular disease in the carotid artery and aorta in women with systemic lupus erythematosus. Arthritis Rheum 2004, 50:151-159. This interesting study dissected the risk factors for both vascular stiffness and for carotid atheroma in SLE. The results suggest that the latter are associated with the classic risk factors seen in the population but that the former is more associated with SLE-specific variables. This could be interpreted as initiation of atherogenesis by disease-related impairment of EC function, with subsequent progression driven by classic factors.

Endothelial cell apoptosis in systemic lupus erythematosus: A common pathway for abnormal vascular function and thrombosis propensity

Rajagopalan S, Somers EC, Brook RD, et al.: Endothelial cell apoptosis in systemic lupus erythematosus: a common pathway for abnormal vascular function and thrombosis propensity. Blood 2004, 103:3677-3683. This paper established that circulating EC particles (and precursors to replace damaged endothelium) are common in diseases with vascular inflammation and that the particles in turn affect endothelial function. Similar EC particles have been reported in acute coronary syndromes. See [54].
