The authors used the most definitive test involving arterial infusions in a clinical physiology laboratory to confirm that ECD occurs in 1°SV, and they related this to disease activity. The improved EC function after therapy may well have related to the clinical remission achieved rather than to a specific effect of TNF blockade.

This study demonstrated increased arterial stiffness that also correlated with disease activity, showing that the altered EC function is associated with clinically relevant changes in blood vessel function. This study and [6,7•] clearly establish the frequency and relevance of EC dysfunction in adult 1°SV.

This interesting study confirmed that the presence of ECD in childhood vasculitis and its persistence for as long as 10 years after the acute stage occurred irrespective of the type of initial therapy. Despite this persistence, the impairment was not fixed because it improved rapidly with ascorbate infusion. This suggests that the chronic maintenance of ECD is an active metabolic process that we are ignorant of at present but that urgently needs unraveling.

This study of Kawasaki disease at adult follow-up examined regional endothelial function in the coronary arteries directly, at catheterization, and correlated this with local myocardial flow reserve using a cold pressor test. Detailed analysis of multiple regions showed impairment according to both test results regardless of a history of aneurysm or current coronary anatomy. This firmly establishes a risk of late cardiovascular disease in all cases of Kawasaki disease.

Arthritis Rheum 2004, 51:447-450. The effects of TNF blockade on EC function are relevant both to understanding the mechanisms of EC dysfunction and to controlling it. This paper suggests a complex response variable with time, as was also noted in an earlier abstract on 1°SV (see [47]). The late loss of benefit apparent at day 28 seems to contradict the data of Booth et al. [7•], but the latter assessed 1°SV patients at 12 weeks, after remission had been obtained. Further detailed serial assessments are needed to differentiate between the direct effects of therapy on EC function and the indirect benefits of suppressing disease activity.

The results suggest that the latter are associated with the classic risk factors seen in the population but that the former is more associated with SLE-specific variables. This could be interpreted as initiation of atherogenesis by disease-related impairment of EC function, with subsequent progression driven by classic factors.
