Yearly residual house spraying performed for 5 years (1993-1998) reduced the abundance of Anopheles funestus plus the availability of antimalarial drugs reduced malaria transmission to very low levels.

A very comprehensive study examining the long-term meteorological trends, including temperature, rainfall, vapour pressure and the number of months suitable for P. falciparum transmission are examined in four high-altitude sites in East Africa. They show that these have not changed significantly during the past century or during the period of reported malaria resurgence. Therefore, the claimed associations between local malaria resurgences and global warming are overly simplistic.

A continuous 30-year monthly malaria incidence dataset (1966-1995) from hospital registers at a Kenyan tea plantation and contemporary monthly meteorological data (1966-1995) from the tea estate meteorological station and from global climatology records were tested for trends. They concluded that whereas malaria admission incidence increased the changes were not attributable to climate changes in western Kenya.

Epidemiological data are presented on 40 deaths (provided by the CDC) between 1992 and 1998.Thirty-eight (95%) of these were caused by P. falciparum and two (5%) were caused by Plasmodium vivax. Anti-malarial chemoprophylaxis was taken in 40% of cases. Twenty-four of the cases (60%) involved US travellers to endemic areas (59% to sub-Saharan Africa, 25% to South America, 8% to India, and 4% to Haiti), 28% involved foreign travellers to the US and three were induced cases (7.5%).

This is the largest retrospective study published to date of imported severe malaria describing the clinical spectrum and factors associated with fatal outcome in adults. It included 188 patients admitted to an intensive care unit in Paris between 1988-1999. Among them, 93 had strictly defined severe malaria. Nearly all infections (94%) were acquired in sub-Saharan Africa.

The latest evidence from post-mortem, in-vitro and animal studies is reviewed in this paper, which highlights the role of BBB breakdown in cerebral malaria. BBB integrity is disturbed during severe malaria, causing leakage of cerebral vessels. This may have major implications for the treatment of severe malaria, in particular the role of neuroprotective therapies and the potential for cerebral oedema in children given volume replacement (see Ref. 36).

The control of seizures was more variable in children given rectal diazepam.

This analysis included studies that compared patients who received adjunct ET with patients who received antimalarial chemotherapy alone for the years 1966-2001. However, there were a number of methodological problems as patients treated with ET were significantly more critically ill; they met a greater number of WHO criteria for severe malaria and had higher levels of parasitaemia than patients who did not receive transfusions, which casts serious doubts on the conclusion of the study.
