Oxygen consumption was measured by respiratory mass spectrometry and estimated using four standard equations in 126 children with congenital heart disease. In patients undergoing cardiac catheterization, VO2 was overestimated by all equations. In children in the intensive care unit, VO2 was significantly underestimated by three equations and underestimated by one equation. Thus, there was poor agreement between measured and estimated oxygen consumption in sedated, mechanically ventilated children with heart disease during cardiac catheterization and after cardiopulmonary bypass.

These data suggest that the beneficial effects of beraprost attenuate with time.

Patients with pulsatile VADs had significantly higher rates of extubation and oral feeding.

Of 145 patients with ventricular failure who received ECLS, 21 were listed for heart transplantation. Of the non-listed patients, 57 patients (46%) were weaned from ECLS and survived. Of the 21 patients listed for heart transplantation, five were weaned from ECLS and did not require heart transplantation, and 12 were successfully bridged to heart transplantation. Of 11 infants, four were weaned from ECLS without heart transplantation, five were bridged to transplantation, and two died.

This study was a retrospective review of the indications and outcome of all patients catheterized on ECMO. Sixty catheterizations were performed in 54 patients. Interventions performed included left heart decompression in 12 patients, catheter-based interventions in six patients, and arrhythmia ablation in three patients. Complications were two myocardial perforations that were treated with pericardial drains. This study shows the feasibility and safety of performing diagnostic and interventional catheterization in a large pediatric tertiary care center.

In a prospective study, cerebral oxygen saturation and blood flow velocity were measured by near-infrared spectroscopy and transcranial Doppler respectively in 34 patients undergoing neonatal aortic arch reconstruction with regional low-flow perfusion. Mean arterial pressure had a poor correlation with the required bypass flow rate. Fourteen of 34 patients had a cerebral oxygen saturation of 95%, placing them at risk for cerebral hyperperfusion if the cerebral oxygen saturation had been used alone to guide bypass flow.
