Lupus 2002, 11:508-513. This study evaluated retrospectively the effect of low- (1 to 1.5 g) and high- (3 to 5 g) dose methylprednisolone in patients with SLE. Despite high- and low-dose methylprednisolone being efficacious in controlling disease activity (lowering of SLE disease activity index (SLEDAI) scores and subsequent prednisolone dose) there were only nine episodes of serious infection in seven patients of the low-dose group compared with 20 episodes in 17 patients from the high-dose group (P = 0.04).

This study evaluated 90 SLE patients with proliferative glomerulonephritis who were assigned to a high-dose IV cyclophosphamide (CYC) regimen (six monthly pulses and two quarterly pulses; doses increased according to the white blood cell count nadir) or a low-dose IV CYC regimen (six fortnightly pulses at a fixed dose of 500 mg), each of which was followed by azathioprine (AZA). The frequency of herpes zoster was twofold higher in patients treated with high-dose cyclophosphamide than in patients treated with low-dose cyclophosphamide.

This study investigated the safety and efficacy of high-dose cyclophosphamide (200 mg/kg) without stem cell transplantation in 14 patients with refractory SLE. All patients received prophylaxis with dapsone, norfloxacin, fluconazole, and valacyclovir. No mortality or fungal infection was noticed.
