In this study, patients diagnosed with DVT or PE were identified from the Danish National Registry between 1977 through 1992 (15,348 patients with DVT and 11,305 patients with PE). A total of 1737 cases of cancer were identified in the DVT group compared with 1372 expected cases, and similar findings were reporte for patients with PE. The risk ratio was substantially elevated only during the first 6 months of follow-up and declined rapidly thereafter to a constant level just above 1.0.

Ann Intern Med (2001) 134(3):191-200. In this study, 900 patients with symptomatic lower extremity DVT (32% also had confirmed PE), were randomized to three treatment arms. The first received UFH infusion, while the second and third group received subcutaneous enoxaparin sodium at two different doses (1.5 mg/kg once daily or 1.0 mg/kg twice daily), along with warfarin therapy. After 3-months of follow-up, symptomatic VTE recurred in 4.1% of the UFH group, 4.4% of the once-daily enoxaparin sodium group and only 2.9% in the group receiving enoxaparin sodium twice-daily. Incidence of major hemorrhage did not direct among the three treatment groups.

N Eng J Med (2001) 344(9):626-631. In this study, 1021 patients with symptomatic VTE (one third had PE) were randomized to fixed dose subcutaneous reviparin sodium (a second generation LMWH) or adjusted dose intravenous UFH. All patients also received coumarin derivative for 12 weeks. At 12 weeks, 5.3% of patients assigned to reviparin sodium had recurrent thromboembolism compared with 4.9% in the UFH. No difference in major bleeding between the two treatment arms occurred. This study confirmed that LMWH is effective and safe as adjusted-dose UFH.

Prevention du Risque d'Embolie Pulmonaire par Interruption Cave Study Group. N Engl J Med (1998) 338(7):409-415. In this randomized and prospective study, 400 patients with proximal DVT, who were at high risk of pulmonary embolism, were randomized to receive vena caval filter or no filter. Al patients received UFH or LMWH. At day 12, the incidence of PE was 1.1% in the group who received filter versus 4.8% for the group who had no filter placed. At two years, 20.8% of the group with the filter had a recurrence of DVT compared with only 11.6% for the no-filter group. This study confirmed that the initial benefit achieved was counter-balanced by the excess in DVT recurrence.
